Medicare Advantage Private Fee for Service (MAPFFS) Plans were established by the Medicare Modernization Act, also established Medicare Part D. The Medicare beneficiary who enrolls with a private company in the MAPFFS pays between $700 and $1,200 per month and the government transfers to the private company its responsibility for the patient's hospital stays, out-patient treatment, or medication.

The idea, apparently, is to privatize Medicare on a voluntary basis.[Citation Needed]
